Application Settings location:

Found under Settings → Applications with multiple tabs across the top.

Application Forms Tab

Contains the main application link (static, does not change).

Confirmations Tab

Controls the confirmation screen shown immediately after submission.

Adds a required checkbox at the start of the application.
