Module 3 - The Family Application Form

Video Summary

  • Navigate to Settings → Applications → Application Forms → Family Application Form to preview and edit the form.

  • The form is fully customizable; you can modify or rebuild it from scratch.

  • There are two field types:

    • Linked fields: Connected to the Family Info Form and usable throughout the system.

    • Unlinked fields: Exist only within the application and are not reused elsewhere.

  • Key decision:

    • Use linked fields for data you’ll need long-term (e.g., ongoing family info).

    • Use unlinked fields for one-time or reference-only information (e.g., acknowledgments, handbook review).

  • To create a linked field:

    1. Add the field in the Family Info Form first.

    2. Return to the application form.

    3. Use “Link a field” to attach it.

  • Deleting fields:

    • Deleting a linked field removes only the link (data remains in the original form).

    • Deleting an unlinked field permanently deletes all associated response data—use caution.

  • Most application fields should be set to “required”, except clearly optional ones (e.g., referrals).
  • Some information (e.g., emergency contacts, authorized pickups) should typically be excluded from the application and collected later during enrollment.

  • You can include Admin-only fields (e.g., internal notes) by setting visibility to exclude Guardians (Guardians can't see it)

  • The family application form should capture shared family-level data (e.g., address) to avoid redundant entry across multiple students.

  • You can add content sections (e.g., handbook text or instructions) using a custom content field.

  • Common method to review and sign a document:

    • Provide instructions or a link to documents (e.g., handbook).

    • Add a signature field (typed name acts as digital signature).

    • Optionally include a date field.

  • Linking to external documents:

    • Insert a hyperlink that opens in a new tab.

    • Allows users to review materials without losing form progress.

  • The form length can vary widely—from very short to multi-page applications with extensive agreements and acknowledgments.
