Module 3 - The Student Application Form

 

Video Summary

  • The Student Application Form is the third form and is completed once per student

  • Includes fields (address, gender, ethnicity, birthday, etc.) linked to the User Info form

  • Can include custom student-specific questions (background, learning style, behavior, challenges, disabilities, etc.)

  • Supports file uploads (e.g., previous school records) with optional instructions/help text

  • Use custom content sections to post the text for documents that you want families to read or for instructions on what to fill out in the form. 

  • Quick note on filling out the application - Applicants can exit and return later via a one-time email link; admins can resend access links if needed

  • Admins can manage incomplete applications from the People/Application page, including:

    • Viewing access links

    • Resending application links via email

  • Advanced settings for each page/section/field:

    • Fields apply to student but are visible/editable by guardians

    • Optional conditional logic available

  • Mark fields as required if responses or uploads are mandatory to prevent incomplete submissions

  • The basic student info is collected upfront (name, academic level, phone, email), prior to the form, so you won't see it on this form
